Please rate per statement, on a scale of 1 to 10, the extent to which you think this statement applies to your organisation. Give a number between 1 and 5 if the statement does not at all or does not apply; give a number between 6 and 10 if the statement does apply or very strongly applies. Filling-in the questionnaire will take you 10 minutes at the most. We will discuss the survey results at our event - Creating High Performance Organisations - on 26th May.
